A Professional Care Manager – a nurse or social worker with a speciality in geriatric care offers expertise in the following areas:
- On call availability 24 hours per day / 7 days per week
- Medication management
- Interviews, matches, orients and trains caregivers
- Advocacy, problem solving, service coordination and representation with health care agencies and other service providers
- Comprehensive assessments including nutrition, safety, depression, mental status, social isolation, gait, ability to function independently.
- Written plan of care and consultation
- Assisted living and nursing home selection and placement
- Communicating and advocating with physicians
- Supervising our caregivers with on-site visits and phone calls
- Education and direct support services
- Discharge planning assistance
- Liaison for out of town family members
