• Do you feel your family member is failing?
• Do you feel he or she is missing meals or forgetting to eat?
• Do you feel your family member is at risk for falls?
• Do you worry for their safety?
• Do you feel your family member is listening more to a friend or neighbor instead of you?
• Does your family member fail to attend medical appointments?
• Does your family forget to take his or her medications?
• Do you feel your family member is taking his or her medications improperly?
• Has he or she visited an emergency room 2 or more times in the last 6 months?
• Does he or she forget or appears confused with routine bill paying?
• Does he or she cry or seem confused when asked to make simple decisions?
• Do you live out of state?
• Do you know whom you will call if you are out of town on business, vacation or in case of an emergency?
